Company statement

Our efforts to reduce incidents and injuries and to improve safety performances are the challenges for the future.

Nobody can ensure that accidents will not occur but we can do our best to reduce the probability that they occur.

We have to accomplish this, but to be successful it is of a paramount importance that ship and shore shares the useful information.

Our goal is simply stated:

ZERO INCIDENTS – ZERO INJURIES

REPORT NEAR MISS AS MUCH AS POSSIBLE

Facts as reported:

During an inspection, while performing the tour in engine spaces, it was noted that the grinding machine in workshop was not fitted with an emergency stop device.

Investigation and root causes:

It was ascertained that the emergency stop device was not fitted to such grinding machine since its design, nor later fitted.

Specific instructions in order to prevent recurrences / Lesson learnt

The captioned grinding machine has been fitted with an emergency stop device.

Safety inspections of working sites performed by Safety Officer and his team are also aimed to detect equipment or machinery failure, misuse or malfunction that could create a hazard situation or condition and involve workers in serious injury.

Any crew member encountering such situation or condition shall report it immediately to the responsible officers.

Proper mitigating and corrective actions must be addressed to control and prevent recurrence.

Please check that equipment and machinery fitted in engine room workshop are provided with its own emergency stop devices and that these are working correctly.

Any non conformity found must be reported to this Office

Please discuss this near miss incident in occasion of you forthcoming safety meeting on board and make also reference to the Chapter 7 of “Code of Safe Working Practices for Merchant Seamen” edited by MCA
